Nursing & Health
The KUSD Nursing and Health Services team is committed to supporting the health, safety and well-being of all students, ensuring they can achieve their fullest potential in the educational environment.
The Health Office may be staffed by a Registered Nurse (RN), Licensed Vocational Nurse (LVN), or Health Technician. There is ongoing communication between our staff to ensure seamless care for your student. If the Health Technician has questions or requires clarification, an RN is always available for consultation.
Immunization Requirements
All Students Entering KUSD
- Board Policy (BP 5141.31) requires immunization records or medical exemption documents
- Students entering 7th grade must have Tetanus, Diphtheria, Pertussis (Tdap) booster
- Immunization records can be faxed to the district nurse at 707-994-1518
Medical Exemptions
- Students with medical conditions preventing immunization may submit medical exemptions
- All exemptions must be issued through CAIR-ME
- Parents can create an account on CAIR-ME and apply online
Whenever possible, medications should be given at home by parents. School personnel will assist with medication administration at school, when necessary for the health and safety of the child, to ensure educational access.
Requirements
- Medication Authorization form completed by a licensed medical provider AND signed by parent/guardian (renewed annually)
- Medications in original prescription or unopened over-the-counter containers
- Proper labeling with child's name, medication name, and dosage
- Direct delivery to the school office or nurse by parent/guardian
Student Medication Policy
Students may not carry prescription or over-the-counter medications except with express knowledge and permission of the school nurse.
Health services prescribed by licensed physicians requiring medically-related training, necessary during the school day to enable school attendance.

- Postural drainage
Required Documentation
- Physician-completed and signed forms:
- Authorization for Specialized Physical Health Care Services Form
- Medication Authorization Form
- Diabetes Medical Management Plan (Type I & II)
- Seizure Action Plan
- Medical Orders from Physician (for catheterization, enteral feedings, other procedures)
Eligibility is provided for students with TEMPORARY disabilities making school attendance impossible or inadvisable due to:
- Temporary illness
- Surgery
- Recent hospitalization
Important Note: Chronic conditions do not qualify for Home and Hospital, but other educational alternatives like independent study may be available.

If there is reasonable suspicion a child may have a contagious or infectious disease, the student will be excluded from attending school until the school nurse is satisfied that the student is no longer contagious, or they are released to return to school by a licensed physician. Follow district guidelines for illness-related absences to prevent the spread of communicable diseases.
