504 Plans
At Konocti Unified School District, we are committed to supporting all students, including those with disabilities. Section 504 of the Rehabilitation Act is a federal law that ensures students with disabilities have equal access to education and school activities. Below is a simple guide to help you understand the 504 process and how we work together to support your child.
What is a 504 Plan?
A 504 Plan provides accommodations and supports to help a student with a disability succeed in the general education classroom. It ensures your child has equal access to learning and school activities.
Step-by-Step 504 Process
1. Referral
Anyone—parent, teacher, school staff—can refer a student for a 504 evaluation.
If you believe your child may need accommodations, contact your school’s principal or counselor.
2. Parent Consent
Before we move forward with an evaluation, we will ask for your written permission.
3. Evaluation
The school team gathers information from multiple sources (academic records, teacher input, medical documentation, etc.) to understand your child’s needs.
This is not a medical diagnosis but an educational review to determine eligibility under Section 504.
4. Eligibility Meeting
You will be invited to a meeting with the school team to discuss whether your child qualifies for a 504 Plan.
To qualify, your child must have a physical or mental impairment that substantially limits one or more major life activities (like learning, concentrating, walking, etc.).
5. Developing the 504 Plan
If eligible, a 504 Plan will be created with input from you and the school team.
The plan will outline specific accommodations or supports your child needs (such as extended time on tests, preferential seating, or behavior supports).
6. Implementation
Teachers and staff will receive a copy of the plan and ensure accommodations are provided.
You can check in with school staff at any time about how your child is doing.
7. Annual Review
The 504 Plan is reviewed at least once a year or whenever needed.
You can request a meeting at any time if you believe changes are necessary.
