Inter-district Attendance Agreement (English and Spanish) |
School Board Policy 5117 provides information on this form. This form is for students wishing to transfer to another school district while living within the Konocti Unified School District area. |

| Home & Hospital Instruction Physician's Form | School Board Policy 6183 - A student with a temporary disability which makes school attendance impossible or inadvisable shall receive individual instruction in the student's home or in a hospital or other residential health facility, excluding state hospitals. A copy of the policy, as well as the form, is included in this document. |

| Oral Heath Assessment Form English | Spanish |
California law, Education Code Section 49452.8, requires that your child have an oral health assessment (dental check-up) by May 31 in either kindergarten or first grade, whichever is his or her first year in public school. Assessments that have happened within the 12 months before your child enters school also meet this requirement. Take the attached Oral Health Assessment/Waiver Request form to the dental office, as it will be needed for your child’s check-up. |
| Consent to Release and Exchange Confidential Information | The Family Educational Rights and Privacy Act (FERPA) applies to "educational agencies and institutions" that receive funds under any program administered by the U.S. Secretary of Education. FERPA protects the privacy interest of parents and students in education records maintained by the agency or institution. By completing this form, which will be maintained at your child’s school, you will allow our district to exchange information regarding your child with local agencies. This will allow us to provide the best educational opportunities possible for your child. |

| Facilities Use Form | Form for Requesting the use of a School district facilities for an event. Please submit the form to the district office. |
| Letter of Resignation Form | Internal form for submitting a resignation or retirement request. |
| Report of Loss or Damage to District Property | If an incident occurs in which damage to property or personal injury is involved, please immediately notify the District Office. Complete this report in its entirety and send to the district office within two days of the incident. District office staff will report to the district’s insurance company if appropriate. |
